Mentoring Support

Guided Futures has a team of enthusiastic, skilled and experienced Mentors, with a variety of life experiences, along with years of experience in working with Participants who have complex disabilities. We provide care and support that is reliable, genuine and non-judgmental. Our team can guide and make community transition safe and smooth for people living with complex
psychosocial disability.

We work with your Allied Health Professional [AHP] to implement your plan. Our mentors will provide feedback on your progress towards your goals so necessary changes can be made along the way. This service can be utilised in various settings, for example, at home or whilst out in the community. We strive to provide the same mentor so there is consistency for you where you need it the most.

Our qualified, experienced team of mentors also provide programs and services that are tried and tested and have proven to be successful with our participants. We acknowledge all participants as individuals and we modify any program to meet the needs of our participants.

Through mentoring support, we can guide you to focus on building new and existing skills. This contributes to building capacity and boosting confidence levels. Our experienced mentors will aim to improve your quality of life by assisting you; our participant, in managing behaviours of concern, complex trauma and communication needs.

Social Activities

The NDIS describes social and recreational supports as being key to accomplishing its objective in
supporting people with a disability to achieve independence, social and economic participation. At Guided Futures, we place emphasis on social activities and it is also one of our specialties. We consider social activities as a part of everyday life. The NDIS encourages and funds for the extra help you may need so you do not miss out because of your disability. 

We describe our programs as fun. They can help with your health and wellbeing and improve your skills to meet new people. We have those fun-loving activities to keep you occupied and learn life skills at the same time. The NDIS is prepared to provide support, whether you need it in the short-term to get started with a program and build your skill level to participate independently, or, to connect with someone who can regularly help you to participate, or, have ongoing support to participate. 

Guided Futures is committed to meeting the needs of our Participants so we can quickly design programs to address special interests.

Some social activities may include things like:

  • Going places for fun
  • Going to the movies or a concert
  • Relaxing, like meditation or yoga
  • Visiting your friends and family
  • Playing sport, such as tennis, cricket, football or basketball
  • Learning new skills, like dance, art classes
  • Recreational activities like kayaking, fishing, rock climbing, hiking, bushwalking, surfing and camping.
  • Life skills like street awareness, using public transport & community markets

In-Home Supports

Guided Futures provide support to enable you to live as independently as possible. We aim to ensure that you live in a safe, healthy environment where you are comfortable and as
independent as you can be. Guided Futures focuses on skill development, our experienced staff will assist you to learn new skills or to maintain your skills to promote independence and work towards achieving your short-term / long-term goals. If you have Core supports funding in your NDIS budget, there is flexibility for choosing supports that meet your individual needs with household tasks.

We assist you with the following tasks:

  • Grocery shopping
  • Personal hygiene: includes showering, dressing and grooming
  • Meal Preparation
  • Kitchen cleaning including sinks, taps and microwaves
  • Budgeting
  • Vacuuming and mopping floors
  • Dusting and wiping surfaces
  • Changing linen
  • Laundry and ironing
  • Lawn mowing and yard maintenance
  • Cleaning toilets, showers and baths

Respite Camps

Guided Futures offers a unique program specifically designed for people with disability, our camps give you the chance to relax, meet new people experiencing similar challenges, and explore new skills in a safe and relaxed setting. Our respite camps also give you an opportunity to challenge yourself, find your hidden talent and get
amongst nature.

Our experienced staff will take care of

  • Your dietary requirements, especially if you are on a restricted diet
  • Any allergies you may have like food and dyes.
  • Medication administration

Get outdoors, find your sense of belonging and form new friendships at our safe, supportive and fun-filled camps.

Support Coordination

Guided Futures employs highly skilled and experienced Support Coordinators who assist you in navigating the disability service system. They assist you in identifying, developing, coordinating and accessing supports and services on your
behalf and help you to access supports and services on their own. Our Support Coordinators focuses on addressing any barriers to accessing disability services and works to resolve them. We assist you with managing multiple or complex supports from a range of providers, monitor your progress and resolve any further issues that may crop up.  Our ultimate goal is to work with you so that you can reach your full potential.

Some of the Support Coordination services we offer

  • Implementing your NDIS Plan.
  • Linking with the community and mainstream supports, such as health services, housing and education.
  • Assist you to understand your NDIS plan and build capacity.
  • Train you how to access the NDIS portal and service agreements.
  • To provide information, advice and support when decisions are to be made in conjunction with your support network.
  • Help you monitor the progress and outcomes of your goals.
  • Help you to prepare for your next NDIS review meeting.

Community Nursing Care

At Guided Futures, our Community Nursing service is structured to support individuals with complex
support needs that require the services of a Registers Nurse within the community. Our experienced Registered Nurses will assess your clinical needs in alliance with you and other health professionals to prepare a Care Plan to meet your care needs. This service can be utilised as part of the Government funded package or privately. Our Person-Centred approach to care will ensure that your independence and general lifestyle will improve, and also limit your visits to the Hospital.

High Intensity Daily Activities

Guided Futures specialises in the provision of High Intensity personal care, we have a pool of experienced and trained staff to deliver high intensity supports. We can deliver these supports in the comfort of your own home or whilst you are accessing the
community. Our services are tailored to suit your needs and can be offered as short or long-term support.

Psychosocial Recovery Coaching

A Psychosocial Recovery Coach is a new support funded by the NDIS for participants who requires support with managing complex challenges of daily living. Psychosocial disability is a disability that may arise from a mental health condition, not everyone who has a mental health condition will have a psychosocial disability. Recovery is defined as achieving an optimal state of personal, social and emotional wellbeing, The NDIS is prepares to help you with your psychosocial recovery goals if you meet the access
requirements.

Our team of qualified and experienced Psychosocial Recovery Coaches are ready to assist you in your
individual journey of recovery. Our coaches have more than 10 years of experience in mental health
related work and have a minimum qualification of Certificate IV in Mental Health.

  • Coaching on Capacity building and resilience through strong relationships.
  • Develop relationships that will assist with recovery
  • Support participants with their recovery plan
  • Coach Participants to increase their recovery skills
  • Motivation, individual strengths and decision-making capabilities
  • Assist with plan implementation
